To make a referral

For your patient to get an appointment with Mr Francis Ting, your patient will need a referral letter. Please ensure your letter has the following:-

Patient information including contact detail and National Health Index (NHI) number
Reason for referral, brief history, examination findings and any investigations completed
ACC 45 number and date of injury if patient has had an injury
Referrer's contact details so we can keep you up to date with our patient's progress

Please discuss any urgent referrals over the phone on (06) 758 0297.

Once referrals are sent through, appointments can be booked by calling our office on (06) 758 0297 from Monday to Friday between 8:30am and 4:30pm. Please also provide your patient with our contact details and feel free to contact our office for more calling cards as required.
